Prevalence and Associated Factors with Maternal Near-Miss among Pregnant Women at Hawassa University Comprehensive Specialized Hospital, Sidama Region, Ethiopia

The first experience of effective 3rd line antiretroviral therapy – a case of 40-year-old female retroviral-infected patient at hawassa university comprehensive specialized hospital, hawassa, sidama, ethiopia, late diagnosis of breast cancer and associated factors among women attending hawassa university comprehensive and specialized hospital southern ethiopia.

Abstract Background; Breast cancer is a significant public health issue in sub-Saharan Africa and the second commonest cancer overall. In Ethiopia, most women present at the late-stage presentation. This is because Ethiopian government gives less attention, and is not well-studied as well. Therefore, it is important to assess delays in diagnosis and treating breast cancer that has been associated with a more advanced stage of the disease and a decrease in patient survival rates. Objective: To assess the magnitude and associated factors for late diagnosis of breast cancer among women attending Hawassa University Comprehensive Specialized Hospital in Southern Ethiopia. Methodology: A facility-based cross-sectional study was conducted from December to January 2019. Data were collected from 261 consecutively selected clients based on the arrival of their hospital visit by using a pretested structured questionnaire and checklist. Physicians performed physical examinations and diagnoses. Data was checked for completeness and consistency, and entered into epi data, then exported to SPSS for analysis. Descriptive, Bivariate, and multivariable logistic regression analyses were performed using SPSS Version 25 Statistical Software. Results: The magnitude of late diagnosis of breast cancer was 86.3%. The woman who had no initial advice for breast biopsy [AOR=5.1, 95% (CI=1.4-18.9)], not sharing the problem to others [AOR=4.7, 95% (CI=1.8-12.2)] and using traditional and faith healers as a first treatment choice [AOR=3.3, 95% (CI=1.2 - 8.8)] were associated with late diagnosis of breast cancer. Conclusions: The majority of women having breast cancer were diagnosed at a late stage. It needs attention to provide better options of the modern health service, and providing accessible initial advice for breast biopsy, and creating awareness about the benefit of sharing problems with family to improve the health of mothers by early diagnosing and managing the breast cancer.

Incidence and predictors of mortality among patients with head injury admitted to hawassa university comprehensive specialized hospital, southern ethiopia: a retrospective follow-up study.

Introduction Head injury is the leading cause of morbidity and mortality throughout the world, especially in resource-limited countries including Ethiopia. However, little is known about the mortality rate and its predictors among these patients in Ethiopia. Thus, the study aims to assess the incidence rate of mortality and its predictors among patients with head injury admitted at Hawassa University Comprehensive Specialized Hospital. Methods Institutional based retrospective follow-up study was conducted among 1220 randomly selected head injury patients admitted from July 2017 to July 2019. Bivariable and multivariable Cox regression models were fitted to identify the predictors of mortality. Proportionality assumption was tested by a global test based on the Schoenfeld residuals test. Results The incidence of the mortality rate was 2.26 (95%CI: 1.9–2.6) per 100-person day observation. The independent predictors of time to death were age above 65 years (AHR:3.49, 95%CI:1.63, 7.48), severe TBI (AHR: 8.8, 95%CI:5.13, 15.0), moderate TBI (AHR:3, 95%CI:1.73,5.31), hypotension (AHR:1.72, 95%CI: 1.11,2.66), hypoxia (AHR:1.92, 95%CI: 1.33,2.76), hyperthermia (AHR:1.8, 95%CI: 1.23,2.63) and hypoglycemic (AHR:1.94, 95%CI: 1.34, 2.81) positively associated with mortality, while underwent neurosurgery was negatively associated with mortality (AHR: 0.25, 95% CI: 0.11,0.53). Conclusion The incidence of mortality rate among head injury patients was high. Older age, moderate and severe TBI, hypotension and hypoxia at admission, neurosurgical procedure, and the episode of hyperthermia and hypoglycemia during hospitalization were the independent predictors of mortality among head injury patients. Therefore, intervention to reduce earlier deaths should focus on the prevention of secondary brain insults.

A Neonatal Hip Septic Arthritis Caused by Klebsiella pneumonia at Hawassa University Comprehensive Specialized Hospital Neonatal Unit, Hawassa, Sidama, Ethiopia

Prevalence of motorcycle accidents and its associated factors among road traffic accident patients in hawassa university comprehensive specialized hospital, 2019, time-to-recovery after cesarean section delivery among women who gave birth through cesarean section at hawassa university comprehensive specialized hospital, south ethiopia: a prospective cohort study.

Abstract Background: Despite the progressive increment of caesarean section rates worldwide over the last decades; still the trend has not been accompanied by significant maternal or perinatal benefits. Moreover, information on the quality of the service, as measured by timely recovery, is scarce. This study assessed predictors of time-to-recovery after cesarean section delivery among women who gave birth through cesarean section at Hawassa University Comprehensive Specialized Hospital (HU-CSH), southern Ethiopia. Methods: Institution based prospective cohort study was conducted on 381 randomly selected women who gave birth by cesarean section in HU-CSH during the follow up period. Pre-tested structured questionnaire was used to collect the data. Data were analyzed using Kaplan Meir (KM) curve, Log rank test and Cox-Proportional hazard model. The outputs of the bivariable and multivariable Cox model are presented using Adjusted Hazard Ratio (AHR) with the respective 95% confidence intervals (CIs).Results: After a maximum of 19 days of stay, 96.2% [95%CI: 94.04-98.4%] of the women were early recovered. The overall median IQR) time of recovery was 2.00 (2, 3) days. The overall incidence density rate (IDR) of recovery in the cohort was 0.34 per Person-days or 2.38 per person-week. On the other hand the overall mean survival time was 3.07(95%CI: 2.75-3.40) days. Women who had ANC follow-up (AHR=1.49, 95%, CI: 1.05-2.10) and discharge from the wound site (AHR=0.13, 95%, CI: 0.03-0.56) were identified as significant positive and negative predictors of time-to- recovery after CS delivery respectively. Conclusion: This study showed that the rate of early recovery was high and is comparable to the global level figures. However, further improving preoperative maternal status, intraoperative follow up and post-operative care is needed to improve early recovery.

Long-Term Immunological and Virological Outcomes in Children Receiving Highly Active Antiretroviral Therapy at Hawassa University College of Medicine and Health Sciences, Southern Ethiopia

Purpose. To determine immunological and virological failure and associated factors among children infected with human immunodeficiency virus receiving antiretroviral treatments at Hawassa University Hospital, Southern Ethiopia. Methods. A hospital-based cross-sectional study was conducted among 273 HIV-infected children from July 1 to December 1, 2019. Data were collected using a structured questionnaire and review of patient records. Blood samples for viral load and CD4 count were collected. Data were analyzed using SPSS version 20. Significance group comparison was done by the Kaplan-Meier log-rank test. The Cox proportional hazard model was used to select significant factors of the variability between groups. Results. A total of 273 children, between the age ranges of 1 to 14 years, were included. Of these, 139 (50.9%) and 134 (49.1%) were males and females, respectively. Children from the rural area were almost five times more vulnerable for virological and immunological failure than those children from the urban area ( AOR = 4.912 , (1.276-8.815), P = 0.032 ). The overall viral load suppression was 196 (71.8%) with a good adherence of 226 (82.9%). Nonsuppressed HIV viral load was found to be 77 (28.2%) which had two times more viral load copies ( AOR = 2.01 , (1.21–2.66), P = 0.001 ) when compared to those who had suppressed viral load copies. The proportions of children who had immunological nonresponse were 45.6% (21 out of 46), 30.4% (14 out of 46), and 23.9% (11 out of 46) among children with baseline CD4 of <200, 201-500, and >500 cells/μl, respectively. Unimproved outcomes among females were noted for immunological and virological failure in this study ( AOR = 1.901 , (1.038-3.481), P = 0.038 ). Conclusion. In conclusion, the highly active antiretroviral treatment appeared highly effective in terms of immunological and virological long-term outcomes. However, viral suppression (71.8%) in our study was far apart from the UNAIDS target of 90% in 2020. For that reason, strengthening adherence counseling and early initiation of HAART is important.

In the study, both qualitative and quantitative research methods were used in order to achieve the objective of the study. Primary data was obtained using questionnaires, unstructured interviews and personal observations. The information gathered through questionnaire from a sample of 140 operators and face-to-face interviews were conducted with MSEs operators. Secondary data was also collected from books, journals, past research works, official documents and the internet. The respondents of MSEs operators were selected by using of Stratified sampling techniques from the study area. Besides, the interview questions were analyzed using descriptive narrations and summation of different respondents’ point of views. The analytical strategy used based on the assumptions developed by previous studies. Data were analyzed using descriptive and inferential statistics with the aid of Statistical Packages for Social (SPSS) for measure of central tendency and dispersion, Pearson’s Product moment correlation Coefficient and multiple regression analysis. On the basis of the findings, the major problems facing the MSEs operators in Gurage zone are lack of finance, lack of management skills and previous industrial experience, lack of sufficient working premises, lack of infrastructural facilities, lack of proper marketing skills and lack of conducive working environments. The results of the findings further indicate that there exists positive strong significant relationship between independent variables and dependent variable. Finally, the selected independent variables may significantly test the variations from the dependent variable at 1% level of significance. Based on the results of the findings, recommendations are forwarded to operators of MSEs , to government bodies and for the other researchers. Key words: Micro and Small Enterprises (MSEs), Factors, Growth and Success.

Abstract The micro and small scale poultry* businesses (MSSPBs) which is manifested as one of the core government policy for economic growth and poverty reduction, has become limited in the face of free marketing and competition imposed by the large scale business (LSB) investors, as well as other microeconomics constraints. This research paper identifies the constraints and challenges of selected MSSPBs in Southern Addis Ababa Kaliti-Akaki zone (SAKAZ). It analyses and examines which factors are more constrain for MSSPBs to lose their business opportunities and to expose them to the decline environment. On the basis of the theoretical framework derived from different literature hypotheses are formulated and then tested in a study through questionnaire presented to the owners of MSSPABs. The major outcome of this thesis study identified and confirmed the concepts described in the literature concerning the failure of MSSPBs in the targeted SAKAZ are due to the challenge of the strategic techniques of the large scale poultry businesses (LSPBs) competitors, financial and other inputs, and managerial and technical knowhow. The business environment of MSSPBs in the egg product operation is analyzed to examine the underlying reasons why MSSPBs sectors encounter these difficulties. The result of the study about the factors contributing to the failure of MSSPBs lead to a couple of suggestions of strategies how MSSPBs could optimize their business development, survive in the business and improve the competitiveness of the sector and recommends a number of business solution which is approved the relevance of the solution by the case study. The analysis of the macroeconomic factors and the industry environment within the egg product operation in SAKAZ, however demonstrates that MSSPBs sectors can only overcome the difficulties they are encountering if the MSSPBs strategic choice towards their business development is tailored to the diversified approaches. To make them again survive in the competition through an alternative strategic diversified approach their capacity building should also be seriously taken into account by the government and other NGOs institution. *Unless and otherwise specified, in this paper context the word poultry means egg production farming.

Writing a research proposal

How to write a research proposal.

For many subjects, writing a research proposal is a key part of your postgraduate research degree application. This is your opportunity to demonstrate your knowledge and how you want to contribute to the subject.

We use the proposal to match your interest with an appropriate supervisor to make sure you have the best support during your degree. We are looking for originality and relevance when assessing the overall quality of your application, including your suitability for this level of study.

We highly recommend that you explore which academic researchers are working in your subject area and contact them first with any questions, this is a good opportunity to firm up your ideas, further explore the topic and talk with others in your field.

What is a research proposal?

A research proposal is a concise and coherent document, usually between 1500 – 2000 words, maximum 4 x A4 pages. You should outline your proposed research project, why it is of relevance (rationale), what research questions are you going to ask, what you hope to achieve (aims and objectives) and how you plan to carry out your research (methodology).

Step-by-step

This page is your comprehensive guide to writing a research proposal and will cover seven key elements of a proposal:

Working title

You should include a title for your thesis in the proposal.

Your title may change as you further your research, but at this stage it's important to state succinctly what your research will cover.

Introduction

Briefly identify your idea, what is your ‘research question’?

It could be the theory you want to test, or a more open question. It would be useful to give examples, 3-5 research questions from recently completed PhDs in a relevant field. You should discuss the context around your research topic, such as current debates and issues. The important thing here is that you introduce your research project with clarity and in a way that stimulates your reader’s interest.

Demonstrate the significance of your research project.

To do this, explain why your research is important, what makes it original and how it will contribute to existing knowledge within its field.

Aims and objectives

What are you hoping to achieve with your research?

Try and produce four or five bullet points of objectives for each aim, which demonstrate your understanding of how to meet your research aims. You can use the SMART acronym to support you in creating objectives, which involves making your objectives: specific, measurable, achievable, realistic and time specific.

Literature Review

Demonstrate your knowledge and awareness of relevant literature

A literature review is a discussion and evaluation of academic literature or a relevant body of knowledge (for practice-based research). You should use this section of your proposal to show that you are familiar with work in your chosen topic area and that your research will contribute something new and/or meaningful to it.

Methodology

Explain how you plan to carry out your research

The methodology section of your research proposal is where you explain how you plan to carry out your research. This should include the research techniques and methods you will use, why these are most appropriate and how you will implement them. You should also include a discussion of the research strategy (general approach) you will adopt, with appropriate justification, including the analytical approach. The section should also contain the range of research findings that will be gathered from the research and how you will analyse or evaluate this. For practice based research, include how will your portfolio of artefacts, code, software, compositions, computer games etc. articulate the originality of your research?

Reference all the materials you used in the preparation your proposal

You may also list references that you didn't directly draw upon, to demonstrate awareness of literature relating to your proposed material.

Support from academic staff in drafting your research proposal

Your research proposal will be read by academics with an interest in your field of research. You are therefore encouraged to contact members of academic staff informally prior to submitting your application to discuss to your research proposal. This can often speed up the applications process, as you can identify the member(s) of staff you have spoken to on your research degree application form.

Use the Huddersfield Research Portal to browse academic staff profiles and search using key words to find staff members who share your research interests.

Changing aspects of your research proposal after gaining a place as a research student

Your research proposal is your starting point, and we understand that as your idea develop s , your proposed research is likely to change. As such, you will not be obliged to adhere to the specifics of your proposal if you are offered a place as a research degree candidate at Huddersfield. However, as the proposal is the foundation of your working relationship with your supervisor(s), you will need to discuss any changes with them first.

Useful tips for writing a research proposal

  • Maintain a focus in your proposal: Your research proposal should be clear and concise, outlining your research idea and its benefits to your chosen field of study, in a way that the reader can clearly understand. Remember, your proposal is just the starting point and an outline and does not need to be overly complicated.
  • Share your proposal: Ask someone you trust (a friend, family member, tutor) to read your proposal and provide some feedback. Do they understand what your research is about? Do they think your aims and objectives are achievable? Does your research engage them?
  • Align your proposal topic with University research themes: Whilst it is important to choose a research topic that you are passionate about, your proposal will be assessed (in part) on its fit with our University research themes. You therefore need to choose a topic which aligns with topics of interest to the University or academic school you hoping to work within and make it clear how your project matches up with them.
  • Be realistic in your proposal: Your proposal is assessed not only on its quality, originality and fit with our research themes but also the likelihood of completion, so make sure that the scope of your research project is reasonable and realistic .
  • Take your time when writing your proposal: There are a lot of elements to a high-quality research proposal, so take the time to ensure that you meet them all. At the University of Huddersfield, there are three opportunities for enrolling onto a research degree programme during the academic year (October, January, and April), meaning less time pressure when working on your proposal and application.

Once you have written your proposal, what next?

Once you have written your research proposal you will need to complete an application form. Look at our how to apply webpage for more information.
